HOW TO APPLY FOR FINANCIAL ASSISATANCE:

  2. Phone calls will delay the response time.
  3. Please communicate by email to check in on the status of your application.
  4. Please remember you are in the screening process
  5. We do require you to pay a portion of your bill and do not pay the entire bill.
  6. Please be detailed in your application so we can understand your crisis.
  7. Respond timely with your application requirements.
  8. Once you are assigned a case manager the fastest response is to email the case manager back.
We care about you and have many applications to process thank you for your patience. 
 

Documents needed to process a request:

  • Client Intake Form- Linked in your email
  • Cleint Budget Form- Linked in your email
  • Valid state ID/Driver's License 
  • Valid social security documentation (showing last 4 digits) for each household member. 
  • Proof of income for all household members
    • Current pay stubs for each person working.
    • Proof of unemployment, SSI or SSA, VA Pension, from family/friends are required to present a statement of income support. 
    • Most recent award letter(s) for SNAP, TANF, or WIC. 
    • Child support documentation.
  • Proof of Crisis (Reason for assistance).
  • For childcare:
    • W-9 for licensed childcare facility 
    • Most recent bill from facility 
  • For ultility assitance:
    • Utility bill/late notice
  • For housing assitance:
    • Lease
    • Late Notice
    • Up-to-date Ledger from landlord
    • Landlord Agreement and Consent Form 
    • Current W9 Form from landlord indicating current calendar year 
    • *Utility/Housing bills must be in the client's name.
